Become a Champion in Action
To apply for the Champions in Action program.
Eligibility:
To be recognized as a "Champion in Action", an organization must:
- Be a Philadelphia-based nonprofit organization that services Philadelphia and addresses community healthcare.
- Provide verification of tax-exempt status under section 501(c)3 of the Internal Revenue Code.
- Have a total operating budget of $5 million or less. Priority consideration will be given to organizations with total annual operating budgets of less than $1 million.
To apply, simply complete the application.
Mail (do not fax) your completed application and required attachments with one photocopy to the address below.
Your application must be received by 5 p.m. on the application deadline: July 18, 2008.
